Losing a tooth can be traumatic for dental patients—affecting their ability to chew, digest, and speak, as well as their self-esteem.1 Implant protocols have been redefined to satisfy patients’ expanding expectations for comfort, aesthetics, and shorter treatment times.2 All of these are excellent reasons to explore immediate implant placement procedures rather than delayed implant placement.3 Utilizing a final titanium abutment at the time of implant placement has also been shown to additionally compress treatment times. 

My introduction to the concept of using definitive abutments at the time of implant placement resulted from a fortuitous circumstance. Following an extraction and an immediate implant in a patient, I discovered that I only had a final titanium abutment instead of a provisional abutment. I placed the final abutment, covered the grafted area with platelet-rich fibrin (PRF) to promote bone and soft-tissue regeneration, prepared and provisionalized the abutment, and recalled the patient one week later.

The peri-implant tissue response looked magnificent. I subsequently used this paradigm for several additional cases, all with the same outcome. These experiences led to an interesting paradigm change for me—and I hope, after reviewing this case report, for my colleagues as well.  

One Abutment-One Time

I am among a group of clinicians who now use a final definitive abutment at the time of surgery. The concept of “one abutment-one time” was the focus of a clinical trial by Canullo et al4 in 2010. The aim of the randomized clinical trial was to evaluate the influence of restoration on marginal bone loss using immediate definitive abutments vs provisional abutments later replaced by final abutments. Dr. Marius Steigmann has noted that foregoing the temporary abutment prevents apical movement of the tissue and, as a consequence, apical resorption of the crestal bone. This actually stabilizes the dentogingival complex, making the soft-tissue architecture more stable both short- and long-term.

The term “one abutment-one time” refers to the connection of an immediate non-removal abutment in a post-extractive implant.5 A study by Grandi et al noted another study’s findings on the concept: “Within the limits of this study, it can be suggested that the non-removal of abutments placed at the time of surgery results in a statistically significant reduction of the crestal bone resorption around the immediately restored implants in cases of partial edentulism….”6

Bone Remodeling

Many factors affect bone remodeling for the success of a dental implant. They include: 

l A microgap at the implant/abutment interface

l Micromovement at the implant-abutment interface

l Microleakage between the implant and the abutment

l Abutment disconnection or reconnection7

CASE REPORT

Socket Shield Technique

This patient was treated using the socket shield technique. The technique involves partial extraction of the tooth while leaving a buccal fragment in situ, followed by the immediate placement of a dental implant. This dentin shield is designed to prevent the buccal cortical bone from resorbing.8 

The case study included in this report involves a 21-year-old, white, female patient. The chief complaint was external resorption of the maxillary right central incisor. Radiographs found an erosive lesion distal to the root of tooth No. 8 (UR central incisor). Clinically, a 5-mm circumferential resorptive lesion was observed below the CEJ that had a hopeless prognosis (Figure 1).

A CBCT scan was obtained to determine the position of the tooth within the alveolus. The tooth was in a Type I position with the root positioned to the facial. This made it an ideal candidate for partial extraction therapy. An extraction was planned with immediate implant placement and a final titanium abutment. The implant used was a Ditron ULT Ultimate 3.75- × 15-mm implant (Ditron Dental USA) with an 11.5-mm, narrow, straight titanium abutment. 

The patient obtained medical clearance for implant surgery. Informed consent for the procedure was reviewed, and the consent form was signed and witnessed. Whole blood was drawn for the fabrication of PRF and for hydration of bone graft material with autologous fibrin. The patient was sterile draped and monitored and had a preoperative chlorhexidine rinse for one minute. Two carpules of 2% carbocaine containing 1:20,000 levonordefrin were administered facially and palatally, and surgery was commenced. 

Using a long diamond bur, the clinical crown was removed to 1 mm above the gingival margin (Figure 2).

The pulpal canal was identified and used as the guide for the socket shield burs that were used sequentially to enlarge the root preparation (Figure 3).

Root cuts were performed at the mesio-palatal and disto-palatal line angles, and a No. 2 surgical length round bur was used to separate the apex from the facial root complex. The palatal root segment was carefully removed from the osteotomy, and the socket shield was thinned and reduced occlusally to 1.5 mm above the parabolic crestal architecture (Figure 4).

An Er,Cr:YSGG laser (Waterlase iPlus [BIOLASE]) was used for final debridement of the osteotomy, removing remnants of the gingival sulcus and periodontal ligament and any potential periapical pathology. A Ditron ULT Ultimate implant (3.75- × 15-mm) was coated with autologous serum to condition the titanium oxide surface (Figure 5) and placed to an ideal axial position within the osteotomy (Figure 6).

A cover screw was placed prior to the grafting procedure to prevent contamination of the internal part of the implant.

The extraction site gap was grafted circumferentially with an OsteoGen (IMPLADENT LTD) bone strip hydrated with autologous plasma (Figure 7). The graft was adapted to the height of the implant body, and the cover crew was removed (Figure 8).

A final 11.5-mm, narrow, straight titanium abutment was placed and torqued to 35 Ncm (Figure 9).

A Teflon plug was placed, and the screw access chamber was sealed with light-cured flowable composite. A final circumferential PRF membrane was placed to fill the soft-tissue defect and protect the gap from contamination during preparation of the abutment (Figure 10).

The titanium abutment was then prepared with a new 558 crosscut fissure bur to ideal parameters (Figure 11). The abutment was then provisionalized with Protemp (3M) resin material and cemented with temporary Implant Cement (Premier Dental).

The implant site was then followed postoperatively for 2 months while the hard- and soft-tissue complex matured (Figures 12 and 13).

Why Specific Implants Were Chosen

For this patient, I chose the Ditron ULT Ultimate because of its excellent initial stability. This implant’s architecture is ideal for creating a stable implant interface at the time of extraction. Two features of this implant particularly concentrate on immediate load stability. According to Ditron USA, the Reverse Concave Neck (RCN) increases the implant contact surface area to preserve marginal bone and soft tissue.8 The taper, concavity, and microthreads of the collar provide atraumatic implant-to-bone contact by decreasing pressure to the cortical bone. The RCN design provides greater stability and improved distribution of the stress to the bone-implant interface. 

In this case, a CBCT scan was taken to evaluate healing and the condition of the socket shield (Figure 14).

The old veneers were removed from teeth Nos. 7, 9, and 10.

Final impressions were then taken for the definitive restorations, and the case was restored 2 weeks later.


DISCUSSION

Prevention of crestal bone loss is an essential element in extraction/immediate implant placement, especially in the aesthetic zone. Selection of an implant system that gives us excellent initial stability and is combined with an abutment connection system that significantly reduces microleakage contributes to success in this paradigm. A precise abutment connection, known as MolecuLock, improves stability by reducing micromovement to further protect the healing tissue complex and contributes to successful osseointegration.

The seal reduces the egress of bacteria, protecting crestal bone and soft-tissue from the risks of peri-implantitis. The biomechanical implant-abutment seal is designed to reduce microgaps to less than 0.5 μm—too narrow for bacteria to penetrate. The implants share one abutment platform: a 2.45-mm internal hex implant-abutment connection across all diameters. 

In addition, the soft-tissue attachment to the definitive titanium abutment actually stabilizes the position of the tissue on the prosthetic interface and also prevents crestal bone resorption as a consequence.


 CLOSING COMMENTS

Staying current on techniques and implant systems that boost initial stability is a key component of maintaining the integrity of the implant site over the long term. While my past protocol involved using a healing or provisional abutment, waiting for the site to heal, and then taking impressions and making the final abutment, to my pleasant surprise, placing the definitive abutment at the time of implant placement shortens treatment time and gives me a better clinical outcome.

In fact, implants are one of the most profitable procedures a dentist can perform. Advertising for implants has ramped up significantly in the last two years as the market has become increasingly competitive. So how can an independent practice effectively stake its claim in the dental implant market?

In our experience, it takes a little creativity and an organized, strategic approach, which we have broken down into six steps.

To start off, it is extremely helpful if your practice already has some dental implant patients because they will be key in spreading the word. Contact your implant patients with the best success stories and invite them in to record video testimonials. A video montage of testimonials in which existing patients explain how dental implants have changed their lives is a highly effective marketing tool and relatively easy to do.

Patients who are seeking information about the procedure want to know that other people just like them have had success. It gives them peace of mind and helps them understand the long-term physical and emotional benefits of implants.

In addition to recording patient testimonials, you should also gather reviews from your implant patients and collect before and after photos to drive the point home even further.

Beyond testimonials and reviews from existing patients, people seeking information about dental implants want to know the benefits of the procedure from the dentist’s perspective. A good way to do this and support your other implant marketing efforts is to write informative blogs and articles.

Make sure to focus on the benefits of implants in general, as well as the benefits over other procedures.

As we say in marketing — features tell, but benefits sell.

In other words, stick to explaining the advantages of implants, without getting too technical. Try to use simple, clear language that anyone could understand, even if they know nothing about dentistry. Leave out the jargon and appeal to patients’ desire to improve their health and change their lives.

The third step in the plan is to create a dental implants landing page for your practice’s website. The purpose of this page will essentially be to advertise the benefits of dental implants. Here, you can include the video testimonials from your patients, the before and after pictures you’ve taken, and the reviews you’ve collected.

There should also be some paragraphs and bullet points explaining why patients should choose dental implants over other treatment options and how implants can have a positive impact on a patient’s health. Include some information about what implants are and the different types your practice offers.

We also recommend including a video of the doctor explaining why they believe in dental implants and how they have benefited their patients.

Once the landing page is up and running, the next step is to start advertising that your practice does dental implants. Using Google to advertise will help you target patients in your specific area who are searching for information about dental implants, tooth replacement options, and dentures.

This is an important step because just having the landing page, a blog, and testimonials does not actually get the practice’s name out there associated with dental implants. You need an advertising service like Google to bridge the gap. Additionally, Google is trackable and provides a variety of options for targeting a specific audience of potential patients.

Another way to get your practice’s name associated with dental implants is to share dental implant related content on social media. Again, here you can use the testimonials, reviews, and photos you’ve already gathered. You can also create original content to keep it fresh. The important thing is to be consistent and post content on a regular basis. Sharing dental implant content on multiple social media channels, as well as on your website, will increase the likelihood that patients searching online will find your blog, your landing page, your reviews, etc.

The last essential step in a successful dental implant marketing strategy is to leverage your existing patient base. Email is an excellent way to reach out to existing patients to tell them about the benefits of dental implants. You’ll want to follow the same guidelines for emails as for blogs, i.e. not getting too technical and focusing mainly on the benefits.

Keep the writing friendly and personal to increase the likelihood that your email will resonate with the patients you are targeting.

If you have patients for whom you have already developed a dental implant treatment plan, reach out to them specifically with an email to remind them why dental implants are their best option and urge them to move forward with their personal treatment plan.

With the six steps outlined above, your practice can become a player in the dental implant market. Whether you implement this plan on your own or work with your trusted marketing partner, you’ll want to be persistent for the best results. With a consistent effort, your practice is sure to see a difference in the number of patients inquiring about implants. And more inquiries means more patients who follow through with treatment and more revenue for your practice.

Dr. Samuel Low is here to answer our questions regarding dental lasers and peri-implantitis, stemming from recent findings from the International Journal of Periodontics & Restorative Dentistry and Lasers in Medical Science.

1. Recently published studies in the International Journal of Periodontics & Restorative Dentistry and Lasers in Medical Science suggest that BIOLASE’s Waterlase laser technology is beneficial for the treatment of peri-Implantitis. Can you summarize these findings?

Data in both journals demonstrates the value of laser technology in treating peri-implantitis. The study published in the International Journal of Periodontics & Restorative Dentistry highlights the value of Er,Cr:YSGG lasers for the debridement of titanium implants. In this study, a laser treatment was carried out at a setting of 1.5 W, air/water of 40%/50%, and pulse rate of 30 Hz. Patients who received the treatment returned with positive clinical outcomes.

Similarly, the findings of the study published in Lasers in Medical Science suggested that the dental lasers reduced the amount of bacteria present on zirconia discs. In addition, fibroblast attachments on the surfaces of the zirconia discs also showed more adherence when treated with Er,Cr:YSGG laser technology. This is important given the uptick in the number of zirconia dental implants placed throughout the world.

In simple terms, both of these studies emphasize the positive role that dental lasers can play in decontaminating surfaces surrounding implants, where elements including biofilm can contribute to peri-implantitis.

2. How can lasers be used to prevent peri-implantitis in the first place?

Before peri-implantitis becomes severe enough to refer to it as such, dental lasers can also play an important role in preventing it in the first place. Lasers can be used to effectively reduce bacteria in the mouth through a process commonly known as laser bacterial reduction (LBR) or Laser Assisted Periodontal Therapy (LAPT). Both have been shown to be successful in minimizing biofilm that can negatively impact a patient’s oral health. Concentrated laser light energy will decrease the amount of bacteria and inflammation. If the area around the implant remains free from these harmful pathogens that can lead to inflammation and infection, peri-implantitis can be prevented or, at the very least, delayed.

3. What about when someone has already developed peri-implantitis? How can lasers be used throughout the healing process?

When peri-implantitis has already become serious, Er,Cr:YSGG all-tissue lasers can be an alternative and effective method for decontaminating the surface of infected implants. More specifically, due to lasers’ unique precision for surface targets, hemostatic effects, and no adverse events of titanium surfaces, periodontists can use laser technology to reduce inflammation around the peri-implant pocket through decontamination.

4. How do these findings impact the patient?

While data varies, we have seen as much as 56% of patients with dental implants who may have peri-implantitis and need treatment, and no patient or practitioner wants to experience the loss of a dental implant. However, some treatment methods can create discomfort, be costly, and possibly be ineffective and unpredictable. However, Waterlase laser technology offers an effective treatment option that is successful, consistent and minimally invasive.

5. What advice would you share with providers who are hesitant about using lasers?

To periodontists, or any dental professional, who is unfamiliar with the use of dental lasers, I would recommend first seeking information, much like these studies. There are a myriad of educational resources available as well as demonstrations and training events to attend to learn more about the benefits of dental laser technology for both the patient and the practice alike. GDIA Live-Patient Surgical Programs in Tijuana, Mexico

This is a dental implant training program designed for dental clinicians to be able to place dental implants and perform implant surgery under close supervision on live patients in Tijuana, Mexico. This course is open to clinicians with or without previous exposure to implant dentistry and does not require prerequisites.

GDIA offers three levels of Dental Implant Surgical Training:
101 Fundamental (25 CE Credits) is for those who have never placed implants before.
201 Intermediate (34 CE Credits) is for those who have placed a few implants but want to grow in confidence and learn new techniques.
301 Advanced (34 CE Credits) is for those who are confident placing implants but want to learn more complicated procedures such as sinus lift.

GDIA also offers:
Soft Tissue Live-Patient Surgical Training Program (38 CE Credits) focuses on soft tissue management rather than implant placement.

All levels will cover from diagnosis, treatment planning and live surgery. With the guidance of instructors and volunteering GDIA mentors, we maintain a 2:1 ratio of participants to instructor. During the program, you will perform surgery on patients under the direction of top clinicians from our faculty members.
